The Center for Medication Management, located at Blanchard Valley Hospital, offers patients a complete disease management program.
Patients are taught take their medications correctly and at the appropriate times reducing the chance of adverse reactions and side effects.
Each patient works one-on-one with a clinician to develop a program based on their needs. They also receive information about their disease, learn how medication affects their recovery, and have their dosages monitored/adjusted in collaboration with their physician. After each visit to the CMM, the patient's primary physician receives a summary report includes current doses, progress notes and laboratory test values.
• Blood draws for select medication monitoring • Monitoring and adjustment of warfarin (Coumadin©) dosages • Cholesterol management • Drug therapy management • Pill box filling • Medication compliance and refill reminders • Monitoring/screening of adverse drug reactions, side effects, and potential medication errors
The clinic is staffed by a registered pharmacist that has a Doctor of Pharmacy degree, residency and/or externship experience in anticoagulation therapy, and advanced training in disease state management.
If You're an Inpatient: Your primary care physician will write an order to enroll you in the program. The CMM pharmacist will handle the rest.
If You Have a Referral from your Primary Care Physician: Your physician’s office must complete the necessary documents and fax them to the clinic. A CMM pharmacist will contact you to arrange your first visit.
If You're Enrolling through a Self-Pay Program: Certain services may be conducted without a referral from your physician and developed to fit your specific needs. Please contact the CMM at 419.423.5177 to arrange these services. |
