For more than 25 years, the services provided by the professional caregivers at Bridge Personal Care Services (Bridge PCS) have assisted countless individuals in remaining comfortable in their own homes while receiving support to be independent and maintain their own self worth.

 

Bridge PCS understands that clients and families are concerned about the quality and trustworthiness of the caregivers that come into their homes and Bridge is concerned about the same things as well. All Bridge PCS nursing assistants have completed an aide training class and have received certification by proving knowledge obtained during the sessions. Many nursing assistants have also successfully completed the test administered by the state of Ohio and are listed in the current State of Ohio Nurse Aide Registry.

All Bridge PCS staff are required to pass a background investigation before being hired and complete an extensive orientation program prior to providing services in the home. Caregivers are also required to complete continuing education throughout the year. We know that maintaining a dependable and well-educated caregiving staff is critical to the success of the Bridge agency! 

 

Services Available

 

Nursing Services include (but are not limited to) the following:

• Geriatric care management*
• RN case management
• In-home physical/environmental assessment to determine plan of care
• Medication management
• Physical assessments and client education
• Blood draws and other lab-related services
• Catheter care

*Geriatric care management includes:
• In-home physical, environmental and psycho-social assessments to determine plan of care
• Complete medication management
• Scheduling and attending doctor's appointments
• Providing and coordinating emotional support
• Providing updates to doctor's and a designated family member or friend
• Aide and/or companion services
• Skilled nursing services
• Physical, occupational and speech therapies
• Foot care, including diabetic foot care
• Arranging for home medical equipment such as walkers, medical alert systems or bedside commodes
• Reducing disruptions in your daily life
• Coordinating additional community support systems

 

Nursing Assistants:

• Assist with all aspects of personal care
• Prepare meals which follow special diets
• Light housekeeping
• Laundry
• Medication reminders
• Collaborate with the nurse to identify the client’s needs
• Completes12 hours of in-service programs annually
• Certified in CPR

 

Companion Services:

• Companionship and support
• Light housekeeping
• Meal preparation
• Medication reminders
• Providing transportation for errands and appointments
• Completed six hours of in-service programs annually

While Bridge PCS is often the sole provider of services in the homes of our clients, our services are also sometimes coupled with home health or hospice services with great success. Personal care services can be provided for as little as two hours a day, up to 24 hours a day.  Services and duration are based upon the needs of the client and payor source requirements if they are present. Payor sources include private pay, long-term care insurance, worker’s compensation, Veteran’s Administration, Medicaid Waiver State Plan and Agency on Aging programs (Passport, Alzheimer’s Care Coordination and Caregiver’s Support Program).
