Greater Northwest Ohio Diabetes Association
Multiple scholarships are awarded annually to provide financial assistance for furthering the education of persons with diabetes. Applicants from the GNODA service area will be considered for the scholarship. The service area includes the following counties: Hancock, Hardin, Putnam, Seneca, Wood and Wyandot. Money will be directed to the student or their respective college or university before the start of classes. The student is required to reimburse the GNODA if a C (or equivalent) or better GPA is not achieved.
Standards for Determining Eligibility of Applicant
- Must have diabetes
- Must be a high school senior or college student
- Renewable annually throughout college enrollment
Scholarship Award Maximum
$250-1,000 per person per year
Application Information
Applications for the GNODA Scholarship may be downloaded from the Blanchard Valley Health System website on January 15 of each year. All applications must be returned by mail no later than March 31. The applicant will complete the designated scholarship form.

Specific Scholarship Requirements:
- Completion of application
- Copy of current transcripts
- 3 letters of reference
- Letter from physician confirming diabetes diagnosis
- Written essay, including description of short and long term goals
- Personal interview with GNODA Board members
Notification of Scholarship Award
Applicants will be notified via phone call or written letter by May 15 whether they have been chosen to be awarded the scholarship.
