Blanchard Valley Health System is transitioning ownership of Independence House, a 50-bed skilled nursing home, in Fostoria, Ohio, to Fostoria Commons LLC, effective December 1, 2025. Fostoria Commons LLC is a family-owned company who specializes in long-term care and senior living. They manage 18 facilities in Ohio and four in Pennsylvania, and will bring the latest best practices in the senior living space to the Fostoria community.
“Fostoria Commons is well-positioned to lead the operations and range of services at Independence House,” said Myron D. Lewis, FACHE, President and Chief Executive Officer of Blanchard Valley Health System. “They will continue to provide the excellent service the facility is known for, and I’m excited about their plans to invest in Fostoria and grow the care available in this area.”
BVHS is working with Fostoria Commons LLC to ensure a seamless transition of care for the residents of Independence House and Independence Place, an attached suite of independent living apartments. Additionally, BVHS is working to help all Independence House associates transition their employment with the new organization.
By identifying a strong partner to build upon these services in Fostoria, BVHS can further focus on the quality patient care our communities need most from their local health system. BVHS will continue to own and operate Birchaven, a skilled nursing and independent living facility that serves seniors living in Hancock County and surrounding counties. Birchaven is part of BVHS’ continuum of care in Findlay, where the organization operates Blanchard Valley Hospital, a licensed, 150-bed acute care facility.
