Blanchard Valley Health System (BVHS) offers comprehensive dementia support services focused on enhancing the quality of life for individuals with dementia and their families. Our teams ensure compassionate, patient-centered care through personalized care plans, memory support programs, and access to skilled professionals. Support services also include caregiver education, respite care options, and community outreach initiatives, fostering a supportive environment for both patients and their loved ones throughout every stage of the dementia journey.
Home Health
Our specialized home health services are designed for individuals experiencing memory loss and require professional care at home.
Who Would Benefit
- Patients needing skilled services from a nurse, physical therapist, occupational therapist, or speech therapist related to memory issues.
- Caregivers can receive support from a trained occupational therapist through the evidence-based Skills2Care® program, teaching strategies to manage a patient’s function as memory loss progresses.
How to Get Started
- Consult your physician or neurologist for a prescription related to Home Health services for memory loss.
- Once the referral is received, our Bridge Intake team will contact the caregiver to schedule services.

Palliative Care
Our Palliative Care services support patients with moderate to severe memory loss, helping manage symptoms and improve their quality of life.
Who Would Benefit
- Individuals with moderate to severe memory loss experiencing difficult-to-manage symptoms.
How to Get Started
- Contact your physician or neurologist for a referral.
- After the referral is submitted, our Palliative Care nursing team will reach out to schedule services.

Hospice
Hospice care offers comfort and compassionate support for individuals with advanced memory loss.
Who Would Benefit
- Patients with advanced memory loss who have limited ability to converse or walk independently.
How to Get Started
- Discuss the possibility of hospice care with your physician or neurologist.
- Upon receiving the referral, our Bridge Intake team will contact the caregiver to arrange services.

Adult Day Services
Our Adult Day Program offers a safe, monitored environment where individuals can engage in stimulating activities while caregivers take a break.
Available: Monday to Friday | 9:00 a.m. – 5:00 p.m.
Daily Rates: Full and half-day options available
Program Features
- Daily art activities and light exercise sessions
- Interaction with child daycare programs

Dementia Caregiver Support Group
Connect with other caregivers for shared support, tips, and encouragement at our monthly Alzheimer's and dementia support group.
