How to Access the MyHealth Patient Portal
Self-enrollment is available for Blanchard Valley Health System patients who are age 18 or older that don't currently have access to the MyHealth patient portal or have never had an invitation emailed to them. Complete this form to determine if you are eligible for the self-enrollment process.
If self-enrollment is not an option for you, the next time you are a patient at Blanchard Valley Hospital, Bluffton Hospital or one of our BVHS medical offices, we will offer to send you an invitation to set up your MyHealth Patient Portal account. We can do this as long as we have your current email address on file. Please provide your email address at the time of your next visit if you are not sure we have it on file.
If you have no upcoming appointments at a Blanchard Valley Health System service, you will need to visit (in person) the BVHS Medical Records/Health Information Services Department located on the first floor of Blanchard Valley Hospital (1900 S. Main St., Findlay), or the first floor of Bluffton Hospital (139 Garau St., Bluffton) or one of our BVHS medical practices or other medical facilities. Our staff will assist you with setting up your MyHealth Patient Portal. In order to protect your privacy and promote health system security, portal access cannot be granted over the phone if you have not recently visited one of our facilities. However, we would be happy to assist you in setting up your access on your next visit if you do not wish to make a special trip to one of our facilities. Please note that if you have not had a visit to a BVHS facility/service on or after October 30, 2016 you will not have any health reports in the MyHealth portal. However, the health maintenance features, such as the Health Tracker, will be available to you.
