Advance Directives
A Living Will or Durable Power of Attorney can ensure that your personal feelings about health care will be honored, even if you become terminally ill, seriously injured or unable to communicate.
Under Ohio Law, a Living Will lets you give advance written directions about your health care decisions. Through a Durable Power of Attorney, you can appoint someone as your agent to make health care decisions for you, should you become unable to make them yourself. Both documents are ways to express your opinions concerning your medical treatment.
All of our patients receive an explanation of their rights under Ohio Law to make decisions regarding their medical care. Living Will and Durable Power of Attorney supplies are available at both Blanchard Valley Hospital and Bluffton Hospital.
