Enrollment Questions

What is MyHealth from BVHS & Cerner?

MyHealth is the BVHS Patient Portal from Cerner. MyHealth offers personalized and secure online access to portions of your medical record. It enables you to securely use the internet to help manage and receive information about your health.

What is Cerner?

Cerner is Blanchard Valley Health System’s Health Care Information System (HCIS). Cerner allows many of our services to all use one computer platform, making electronic communication between departments much easier. With this system comes a patient portal called, MyHealth.

What services can I access with the MyHealth Patient Portal?

Your health is important to you around the clock - not just during office hours. With MyHealth, you can connect to Blanchard Valley Health System online to:

  •  Request appointments from Blanchard Valley Medical Practice providers & screening mammograms

  •  Request prescription refills from BVHS providers

  •  Review your medications with instructions

  •  Review your visit history for appointments at BVHS facilities occurring on or after October 30, 2016.

  •  Review your hospital test results such as lab and radiology results

  •  View immunization records

  •  Send and receive secure messages to and from your BVHS health care team

  •  Update your personal information

How do I set up an account on the MyHealth Patient Portal?

Self-enrollment is available for Blanchard Valley Health System patients who are age 18 or older that don't currently have access to the MyHealth patient portal or have never had an invitation emailed to them. Complete this form to determine if you are eligible for the self-enrollment process.

If self-enrollment is not an option for you, the next time you are a patient at Blanchard Valley Hospital, Bluffton Hospital or one of our BVHS medical offices, we will offer to send you an invite to set up your MyHealth Patient Portal account. We can do this as long as we have your current email address on file. Please provide your email address at the time of your next visit if you are not sure we have it on file. After your discharge from the hospital or after your office visit, a secure email will be sent to your email address. We encourage you to log in when you arrive home to be sure your information is correct and to ensure that your MyHealth Patient Portal is ready when you need it! You will have 90 days to log in. If you do not log in within 90 days, you will need to contact our help desk at 1.877.621.8014 to have a new link to MyHealth emailed to you.

What if I have no upcoming appointments/scheduled hospitalizations but want to sign up for the MyHealth Patient Portal?

You will need to visit (in person) the BVHS Medical Records/Health Information Services Department located on the first floor of Blanchard Valley Hospital (1900 S. Main St., Findlay), or the first floor of Bluffton Hospital (139 Garau St., Bluffton) or one of our BVHS medical practices or other medical facilities. Our staff will assist you with setting up your MyHealth Patient Portal. In order to protect your privacy and promote health system security, portal access cannot be granted over the phone if you have not recently visited one of our facilities. However, we would be happy to assist you in setting up your access on your next visit if you do not wish to make a special trip to one of our facilities. Please note that if you have not had a visit to a BVHS facility/service on or after October 30, 2016, you will not have any health reports in the MyHealth portal. However, the health maintenance features, such as the Health Tracker, will be available to you.

The activation code that BVHS sent me in my email does not work. What should I do?

For your security, your temporary activation code expires after 90 days if you do not activate your account. Also, once you activate your account, the activation code will no longer work. An activation code cannot be reused. If you need a new activation code, call 1.877.621.8014.

Is there an App I can download for my smartphone and/or tablet?

Yes, the App is called “HealtheLife.” Go to the App Store on your mobile device, search for “HealtheLife,” and download it for free. The MyHealth Patient Portal can be accessed directly through your web browser. If you use the HealtheLife app, you will receive notifications about your health record and upcoming appointments, etc. directly to your mobile device.

Proxy Information

What is proxy access?

Proxy access allows parents, legal guardians or caregivers to access a patient’s health information on the patient portal. Proxy access is an available option for adult patients (18 years) and pediatric patients (newborn – 12 years of age). Proxy access can be granted by the patient or their legal representative by completing the Proxy Access Authorization form and must be done in person or by sending the notarized form to Blanchard Valley Hospital, Bluffton Hospital or a Blanchard Valley Medical Practice office.

How do I gain proxy access to view a family member’s health record through the MyHealth Patient Portal?

In order to gain access to a family member’s patient portal health record, complete the appropriate adult or pediatric minor “Proxy Access Authorization Form” and return it and any requested documentation in person to the BVHS Health Information Services Department located on the first floor of Blanchard Valley Hospital (1900 S. Main St., Findlay), the first floor of Bluffton Hospital (139 Garau St., Bluffton), or at a Blanchard Valley Medical Practice provider office where the patient is being seen. Access cannot be granted online. It must be completed in person or by sending the form, which is notarized. This form can be obtained at (www.bvhealthsystem.org/myhealth) or at any of the above locations.

Will I be able to see my children’s health information on the MyHealth Patient Portal?

Yes, you will have access to your children’s health information through the MyHealth Patient Portal from birth through 12 years of age if a pediatric minor proxy form has been completed. When a child becomes 13, record access will automatically be revoked.

In order to be in compliance with the state of Ohio law, proxy access will not be provided to adolescents between the ages of 13 - 17. The adolescent patient may directly enroll in the patient portal unless restricted by their provider. When a child reaches age 18, they are considered an adult and can give access to another adult by completing the proxy access process.

If I want to add all of my children under 12 do I need a different form for each child or can I put it all on one form?

No, the form allows for three children to be added to the same form. The only time you would need more than one form is if you have more than three children.

Why is proxy access severed when a minor reaches age 13?

The state of Ohio has laws specific to the health care of adolescent patients between 13 - 17 years of age. In order to support compliance with these laws, proxy access is not available after age 13 until the patient reaches 18 years of age and is able to authorize access. The adolescent patient may directly enroll in their MyHealth Patient Portal, unless restricted by their health care provider.

Do divorced parents have an equal right to view their children’s medical information on the MyHealth Patient Portal?

Yes, both legally-recognized parents have an equal right to proxy access of their children’s health information on the MyHealth Patient Portal (from birth through age 12 years) regardless of who has custody of the child unless there is a court order or legal proceeding prohibiting the rights of a parent.

What is the process for removing proxy access (other than for minors)?

An adult patient or their legal representative may remove proxy access for any reason and at any time. The Revoke Proxy Access form must be completed and submitted to Blanchard Valley Hospital, Bluffton Hospital or at one of the Blanchard Valley Medical Practice provider offices. BVHS also reserves the right to remove proxy access at their sole discretion if there is suspected abuse or negligent use. This form can be obtained on BVHS’s website or at any of the above locations.

Is there a guest option for users to access the MyHealth Patient Portal?

There is no guest option for the MyHealth Patient Portal. Any user that has access to a patient’s record would need to have access as a proxy user.

Your Medical Information & Communication with Your Medical Team

Will my past history of medical information be available on the new MyHealth Patient Portal?

No, only health information gathered on or after October 30, 2016, will be available on the MyHealth Patient Portal. MyHealth is part of BVHS’s new electronic medical system called Cerner. Because it is a new system, the only information gathered after installation will appear on MyHealth. While information prior to October 30, 2016, is stored in your hospital medical record, your MyHealth Patient Portal will display information added after the creation of the portal.

What information is included in the “Medication” section of the MyHealth Patient Portal?

The medication area of the MyHealth Patient Portal provides you with a way to view your active medications, access information about the drugs you are taking and request medication renewal. Medications may be accessed directly from the health record.

Why are some of my medications not listed on the MyHealth Patient Portal?

Medications listed on your health record in the MyHealth Patient Portal are those that are prescribed to you by a BVHS owned and operated medical office or prescribed while at one of our facilities (Blanchard Valley Hospital, Bluffton Hospital, Physicians Plus, etc.). If you receive care at a medical office that is not owned and operated by BVHS, then medications prescribed by physicians at those facilities will not be included on your patient portal. Medications are updated on MyHealth each time you are a patient. So if you were prescribed a short-term medication such as an antibiotic, that may not be included in your current list of medications. Also, old medications cannot be removed but will be updated at your next visit.

Is my physician visit information available on the MyHealth Patient Portal?

If you receive care at a BVHS owned and operated medical office, your provider’s care notes will be included in the patient portal. Only BVHS medical offices and physicians employed by Blanchard Valley Health System are linked to the MyHealth Patient Portal. For a list of offices that are owned and operated by BVHS please visit www.bvmp.org.

Will care given to residents at BVHS’s long-term care facilities Birchaven Village and Independence House, as well as Bridge Home Health & Hospice, be included on the MyHealth Patient Portal?

No, only care received at Blanchard Valley Hospital, Bluffton Hospital, The Armes Family Cancer Care Center, Sak Sleep Wellness Center, Physicians Plus, Wound Care Solutions and BVHS-owned medical offices will be included on the MyHealth Patient Portal.

When can I see my test results using the MyHealth Patient Portal?

Your test results are released to your account once they have been finalized. This typically takes about three days.

Can I view or send messages to my BVHS doctor or nurse using the MyHealth Patient Portal?

Yes, you will be able to communicate via secure online messages. You will also receive portal notifications in the messaging inbox.

Note: Communication with your medical team can only be done using the MyHealth Patient Portal. The previous BVHS Patient Portal is only available to view medical history prior to October 30, 2016.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within three business days. Please note that the MyHealth Patient Portal should not be used for urgent situations. Please go to your nearest medical center if the situation requires immediate attention or dial 911 if it’s an emergency.

Can I request a prescription refill using the MyHealth Patient Portal?

Yes, you can ask your provider for a refill in the medication section of your health record. If your prescription has no remaining refills, choose “renew” a prescription to request a renewal.

What should I do if I don’t understand my health information?

You can discuss any questions with your provider at your next office visit or call their office for explanations.

My Appointments

Can I request an appointment using the MyHealth Patient Portal?

Yes, this can be done through MyHealth for services that do not require a physician’s order. You can request an appointment in the Appointments section. Simply choose the “Schedule a New Appointment” link and follow the prompts. Appointment requests may only be used to schedule wellness visits with Blanchard Valley Medical Practice providers and mammograms.

Once you make an appointment request, it will not show on your portal until the appointment has been scheduled by your medical team. Once the appointment has been scheduled, you will see the appointment listed under “View upcoming appointments.” You will also receive an e-mail notification that your appointment has been scheduled if you have your settings configured to receive email notifications.

What information will show within the “Upcoming Appointments” section on MyHealth Patient Portal?

All appointments that have been successfully booked will appear within the “Upcoming Appointments” list on your portal. If you have ‘pending’ appointments that have not yet been scheduled, these will display within the ‘Appointments’ section of your portal showing a pending status.

Updating Information/Paying Bills

Where can I update my personal information such as my home address and phone number?

You can update information such as your address, marital status, etc. in the "Account" section on your portal. Access this section from your MyHealth Patient Portal home page. Click the dropdown next to your name in the upper right corner of your homepage. Choose “Account.”  If you have a question about the information in your medical record, please contact your provider’s office or call the MyHealth Patient Portal Help Line at 1.877.621.8014.

Where can I update my password?

You can update your password in the “Sign In And Security” section on your portal. You may access this section from your MyHealth Patient Portal home page. Click the dropdown next to your name in the upper right corner of your homepage. Choose “Sign In And Security” And then click “Edit” next to password. Follow the prompts to create a new password.

How can my preferred pharmacy be changed?

When you request a prescription refill through the portal, you have the opportunity to select your current preferred pharmacy or choose a new pharmacy by selecting the “other” option. Your medication will be sent to the new pharmacy you choose and that location will become your new preferred pharmacy for that medication.

Can I pay my hospital bill through the MyHealth Patient Portal?

Yes, currently this feature is available on the home page of MyHealth or at bvhealthsystem.org. There will be two choices to pay your bill. 

Why are my results displaying in the metric system such as Celsius and Kilograms?

Your medical team uses the metric system as a standard of care. MyHealth pulls your medical record directly from the computer system that your health care team uses to log and store your information. There is a link on the homepage of your MyHealth to convert those results to pounds, Fahrenheit, etc.

General Questions About the Availability of Your Health Record

Why can I only see information from recent BVHS medical visits on the MyHealth Patient Portal?

The MyHealth Patient Portal contains information about BVHS provider visits, hospital stays and tests that happened on or after October 30, 2016. While information prior to October 30, 2016, is stored in your hospital medical record, your MyHealth Patient Portal reports and results only display information added after the creation of the portal.

Why are my mental health records not displaying on my MyHealth Patient Portal?

Due to the sensitive nature of mental health records, they will not display on your patient portal. If you'd like a copy of your records please contact your provider or the BVHS Health Information Services department (medical records) located on the first floor of Blanchard Valley Hospital (1900 S. Main St., Findlay).

Technical Questions

Is my medical information secure?

Security measures within the MyHealth Patient Portal prevent unauthorized persons from reading communications. Health records and information can only be read by someone who knows the correct username and password to log in to your portal site.

What is the BVHS Privacy Policy?

Blanchard Valley Health System is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records and will never be sold or leased by Blanchard Valley Health System and its affiliates.

Do I need any special computer equipment to use the MyHealth Patient Portal?

No, all you need is access to a computer or mobile device, an email account and an internet connection.

What happens if I forget my password?

If you forget your password, you will be able to go into the portal, click on “Forgot Password” and enter answers to a series of questions (that you previously responded to). You will then be assigned a one-time password. Once you sign in with the one-time password, you will be prompted to enter a new one. If you are having technical difficulties, please call the MyHealth Portal Help Line at 1.877.621.8014. For security reasons, the Help Line cannot give you a new password over the phone, only explain to you how to reset.

Does the portal have an automatic timeout?

Yes, if you are logged in to your portal with no activity for 30 minutes, you will be logged out for your security.

Can I ask emergency-related questions via the MyHealth Patient Portal?

No, the MyHealth Patient Portal should not be used for emergency-related health care issues. If you are experiencing a health care emergency, call 911 or visit the emergency room.

How do I cancel my patient portal account?

Call the MyHealth Patient Portal Help Line at 1.877.621.8014 to request that an administrator cancel your account.

Can I view my portal on my cell phone or tablet?

Yes, you can log onto the website just as you can with a desktop computer. There is also an App is called “HealtheLife” to use on your smartphone or tablet. Go to the App Store on your mobile device and search for “HealtheLife” and download for free. If you use the HealtheLife app you will receive notifications about your health record and upcoming appointments, etc. The App is not required to view MyHealth on your mobile device.

Language and Special Needs Availability Questions

Can Spanish-speaking patients use the portal?

Yes, your home computer browser settings can be changed to Spanish which allows the MyHealth Patient Portal to display in Spanish.

Tools to Manage Your Health

In the Health Tracker tool, is my health data automatically pulled into the tool from my medical chart?

No, the Health Tracker is designed for you to manually enter your daily weight and goals. A link to the Health Tracker can be found on the homepage of your MyHealth Patient Portal.

What are the workshops?

The workshop section of your MyHealth Patient Portal may list health information and educational videos your medical team suggests you should complete. A link to the workshop section can be found on the homepage of your MyHealth Patient Portal.

What is the MyHealth "Clipboard?"

The MyHealth “Clipboards” allows patients with upcoming appointments to enter and review information prior to their appointment, including concerns and details specific to their visits such as medication, allergies, health issues, surgeries and general health questions. This is a new feature under development in the MyHealth portal. Not every patient will receive a clipboard. BVHS will be expanding this feature to include more patients as the program develops.
 
Currently, patients with young children will be receiving the online clipboards as well as patients with well care visits. An associate from the provider’s office will inform the parents and caregivers of whether they can expect a Clipboard. As parents and caregivers schedule an appointment with a BVMP provider, an email will be sent with a link to complete a MyHealth Clipboard prior to their appointment. Any information included on the form will be available to their provider at the time of the visit. Additionally, the provider office can send pre-appointment clipboards for parents and caregivers to complete prior to their arrival. This reduces the amount of time patients spend filling out paperwork at the office and allows the patient to have more time with the provider.
