Update Your Health Information Exchange Consent Status

A Health Information Exchange allows physicians and providers to securely share patients' medical records electronically. Patients are automatically enrolled in the Health Information Exchanges. To opt out or rejoin, complete and submit the online form below.

This supports better care coordination, reduces duplication, and improves patient outcomes. When enrolled, providers can access information such as allergies, medications, immunizations, lab results, diagnoses, and treatment records from other participating sites nationwide.

CommonWell Health Information Exchange (HIE) is a nationwide health data sharing network.

CliniSync HIE is Ohio’s statewide Health Information Exchange.
