Patients of FSC
Before your surgery, we will verify benefits with your insurance company and discuss your individual finance and insurance considerations if necessary. We participate with most local and national commercial insurers. It is your responsibility to notify your insurance company of your upcoming procedure and to confirm whether we are an in-network or out-of-network facility. Worker’s Compensation claims require more information. If your surgery is the result of an accident at work, please tell your physician’s office.
As a courtesy to you, we will submit medical claims to your insurance company. After the claim is processed and insurance has been applied, you will receive a bill/statement. This bill includes the facility fee for the use of our Surgery Center. You are responsible for any deductibles, co-insurance or balance not covered by your insurance company.
Procedure Price Look-up
Additionally, you will receive separate bills from the following providers (if applicable):
• Your surgeon
• Your anesthesiologist
• Pathology, if this service is used (Blanchard Valley Pathology)
If you do not have insurance, or if you are having cosmetic surgery, you will be required to pay for your surgery two weeks prior to your procedure, unless other arrangements are made with our business office. A price quote will be made available to you based on the information provided by your surgeon’s office. The final bill may be higher or lower based on the actual procedure(s) performed.
Financial Assistance
We realize some patients need a little help making their procedure more affordable. We offer several payment options, including payment plans through Findlay Surgery Center or CareCredit, a deferred interest medical credit card. Be sure to ask your physician if they accept CareCredit as well.

There are several options available for payment plans through our facility. The acceptable forms of payment are cash, cashier’s check, credit or debit card, Care Credit, or personal check.
