Who is eligible to receive a COVID-19 booster shot
To receive the COVID-19 vaccine by appointment, please call Hancock Public Health: 419.424.7441 to make an appointment.
At select Blanchard Valley Medical Practices (BVMP)
Adult COVID-19 vaccines (dose one, dose two and boosters for ages 18 and greater) are available for established patients of the following Blanchard Valley Medical Practices (BVMP) primary care practices. Please call the office to schedule.
Pediatric COVID-19 Vaccines are available for established patients at Caughman Health Center. Call 419.427.0809 for an appointment.

When To Call Your Local Health Department
Please call your local health department for the following reasons:
- If you cannot locate your vaccine appointment card
- If you cannot make it to your scheduled vaccine time
- Those who need help registering for a COVID-19 vaccine
Specific information, such as what to bring and what to wear, will be provided when appointments are made.

Please do not call your local hospital directly to set-up appointments to receive your COVID-19 vaccine. Hospital switchboards are being overwhelmed with phone calls, which is creating an obstacle to patient care. Also, do not email questions pertaining to scheduling issues through the email below. This email is for general education and information, not individual vaccine scheduling.
