At select Blanchard Valley Medical Practes (BVMP)

Adult COVID-19 vaccines (dose one, dose two and boosters for ages 18 and greater) are available for established patients of the following Blanchard Valley Medical Practices (BVMP) primary care practices. Please call the office to schedule.

Pediatric COVID-19 Vaccines (children ages 5 to 17) are available for established patients at Caughman Health Center. Call 419.427.0809 for an appointment.

Allen County Public Health appointments can be scheduled on Mondays and Fridays by calling 419.228.4636.

Blanchard Valley Health System (BVHS), along with healthcare providers across the nation, are experiencing a shortage of rapid COVID-19 testing supplies directly related to the nationwide surge in COVID-19 cases. Other testing options are available - however, there will be at least a 24-hour turnaround time for testing results. If you have a primary care provider, please contact your provider for potential testing options through the offices. Please do not call BVHS facilities with questions regarding rapid testing availability, as this takes our healthcare providers away from the valuable care they are providing to other patients.We are aware that the phone number for the COVID-19 vaccine clinics, 419.424.7105, is experiencing a high volume of callers. This number is specifically for Hancock Public Health. To contact other local health departments, please click on the appropriate county link below. Staff members continue to work on expanding resources available to assist customers. Telephone calls are answered from 8 am until 4:30 pm, Monday through Friday.  

The scheduling for vaccine clinics is dictated by the number of vaccines we receive from the State of Ohio. 

When To Call Your Local Health Department

Please call your local health department for the following reasons: 

  • If you cannot locate your vaccine appointment card 
  • If you cannot make it to your scheduled vaccine time  
  • Those who need help registering for a COVID-19 vaccine 

Specific information, such as what to bring and what to wear, will be provided when appointments are made.  

We appreciate your patience. Your local health department will provide additional information. Please note, we kindly ask that you do not bring children who are not receiving the COVID-19 vaccine to the clinics. Blanchard Valley Health System discourages children to be left unattended in the lobbies and child care will not be provided.

Please do not call your local hospital directly to set-up appointments to receive your COVID-19 vaccine. Hospital switchboards are being overwhelmed with phone calls, which is creating an obstacle to patient care. Also, do not email questions pertaining to scheduling issues through the email below. This email is for general education and information, not individual vaccine scheduling.
