Bridge Palliative Care professionals focus on providing care to those with life-altering illness. Our team specializes in treatment options, pain management and symptom management in the home, at a nursing home, in assisted living and/or at the hospital.

The goal of palliative care is to prevent and relieve suffering and to support the best quality of life for patients and their families. Palliative care is centered on physical, emotional, social and spiritual care. We provide relief of symptoms, help you make the most of every day and, if necessary, help prepare for a peaceful death.

Usually, an early referral is best.  

Our support services can be very helpful when started soon after the diagnosis of a chronic illness. Our goal is to treat distressing symptoms and support you and your family as difficult medical care decisions are made.
